Display/hide an entire custom form by project

Custom forms (and even Custom Attributes) should have the ability to be applied to a "project type." For example, a specific form may be relevant for a for a restoration project type but not relevant to a Remodel project. Therefore, if a custom form has default values added in to text fields, those form elements automatically are being displayed on PDF exports of reports.
Another way to address this could be via a toggle button to display/hide an entire custom form by project. This would allow using custom forms (and even custom attributes) to create templates of regularly used text entered in as default values to show on Reports. For example, this would be advantageous for showing certain disclaimers in some reports vs. others where they may not be necessary depending on the type of project.
